This is the Index page for finding the forms you will need to do your Group Status Reports.
Click on the appropriate link to locate the Form you require. The forms are done in Microsoft Word format and in PDF format. You will need Adobe Acrobat Reader, available HERE in order to read and print the PDF format. Please note: you cannot fill the reports in, on line if you use the PDF format. You must print them, fill them in and then Fax or mail them to the EFM Canada Office at the address below.
If you download the Word document, you may fill it in on your computer and send it to the EFM Canada Office as an E-mail attachment.
